THIS IS REQUIRED by the FDA and we cannot deliver your order without it. 

Our U.S. certified pharmacy will need to verify your identity, in order to mail prescription medication. Your information is kept private and secure, as our servers are HIPAA compliant. You can use your phone to take a picture of the front side of your ID, or make a photo copy of your ID if you want to fax it. You may use some tape or piece of paper to block out your ID number. Just make sure your photo, full name and date of birth are clearly visible. For example, please see below.
